HOW TO SCHEDULE AN APPOINTMENT?

Appointments are scheduled

Monday - Friday from 8:00am to 12:30pm and 2:00 pm - 4:30 pm.

 

There are two ways to schedule an appointment. By Online Booking or By Phone.

You can book an appointment via our Booking Page.

If scheduling by phone, please contact either office to schedule an appointment.

If you are unable to keep an appointment, please give us at least 24 hours notice so that we may give your appointment time to another patient.

WHAT ITEMS TO BRING TO APPOINTMENTS?

  1. Current insurance card(s)

  2. Office visit co-pay and/or balance payment

WHAT’S THE POLICY ON PRESCRIPTION REFILLS?

Prescription refills require 48 hours to process.


In order to process any refill request, patients are required to have had an office visit within a 3-6 months period. 


An office visit is required to fill all prescription refills that have expired.

Please note that refill requests for narcotics, antibiotics or psych medications are only performed during an office visit and only after a medical evaluation by a doctor or nurse practitioner.

CAN I GET A SAME-DAY APPOINTMENT? DO YOU OFFER WALK-IN CLINIC SERVICES?

Yes, we accept same-day and walk-in appointments.

WHAT’S THE POLICY ON REFERRALS?

Referrals to another office or specialist require 2 days to process. 


Failure to get the required referral could cause denial by your insurance provider.

WHAT IS THE AGE RANGE OF YOUR PATIENTS?

At the GoodLife Medical Center, we serve all patients 16 years and older.
